A registry is a research program that collects health information from people who share a specific condition or experience. The goal is to better understand the condition, improve research, and support the development of future treatments or care approaches. 

You can access the registry by downloading the Health Storylines app and creating an account. Once registered, you will be prompted to enter the study code 5TGKGN to join the registry.

Your diagnosis is self reported. You may report your diagnosis via the enrollment questionnaire in your Health Storylines app. 

After completing the enrollment questionnaire and validating your diagnosis, you are eligible to receive $100. There may also be incentives in the form of compensation or raffle draws for subsequent questionnaire completion. If incentives are available, this information will be clearly stated via email.

Surveys will be available on a monthly basis. Participation in each survey is optional.

If a survey includes compensation, payment details and delivery method will be shared within the survey instructions. Payments are typically provided after survey completion. 

This usually means the study code was not entered correctly. Go to your homepage and navigate to your profile section. Scroll all the way down until you see 'Organizations'. Under that field, type in and select 'AXIS'. If you still have issues seeing the survey, please reach out to support at [email] in order to participate in the registry.

Your data is managed by the registry research team and the platform hosting the registry. All information is stored securely and handled in accordance with privacy and data protection regulations. You will find all information about how your data is managed in the Informed consent you will review before entering the registry. 

Yes. Participation is completely voluntary. You can stop participating at any time by discontinuing surveys or withdrawing through the app without any penalty.
